We invite non-governmental organizations to participate in the elections of representatives to the Warsaw Council for Public Benefit Activities for the 2026-2029 term. Non-governmental organizations operating in the territory of the Capital City of Warsaw are entitled to vote. From 21 September to 12 October 2026, voting is underway in the elections of representatives of non-governmental organizations to the Warsaw Council for Public Benefit Activities. Before casting your vote, familiarize yourself with the information clause on the processing of personal data. Instructions for correctly completing the form for selecting candidates to the Warsaw Council for Public Benefit Activities for the 2026-2029 term For the vote to be valid, the form must be completed correctly and submitted within the deadline indicated in the form. The form is valid only when all formal requirements are met. Selection of candidates Mark with an "X" or by checking the checkbox a maximum of 12 candidacies from the list of candidates. Marking more than 12 candidates renders the form invalid. Organization data The following must be entered: the full name of the organization, the KRS number or the number of another relevant register or record (along with an indication of the type of register, if it is not KRS). Declaration of conducting activity in the territory of Warsaw Note. Only organizations conducting activity in the territory of the Capital City of Warsaw have the right to vote If the condition is met, check the box: ☐ "I/We declare that the organization operates in the territory of Warsaw." Signatures of persons authorized to represent the organization The form must be signed by the person or persons authorized to make declarations of intent on behalf of the organization in accordance with the rules of representation resulting from the KRS, another register, or the organization's statute – if this information is not publicly available, attach a document confirming the manner of representation. A valid signature is considered to be: a qualified signature, made by the person or persons authorized to represent the organization, a trusted signature, made by the person or persons authorized to represent the organization, a handwritten, legible signature enabling the reading of the first and last name of the person signing. If the handwritten signature is not fully legible, it is recommended to: place a name stamp above the signature, or handwrite the first and last name in block letters above the signature place. Such marking allows for unambiguous identification of the person making the signature. Date - the date of signing the form must be filled in Information regarding the submission of electronic signatures In the case of submitting the form using a qualified signature or trusted signature, the location of the graphical visualization of the signature on the document does not matter. What is important, however, is that the signature is correctly saved in the electronic layer of the document. This means that: the signature image itself or a screenshot does not constitute a valid electronic signature, after signing, the document must retain its electronic form and allow verification of the signature, do not print an electronically signed document and then scan it and send it as a new file, do not take screenshots of the signed document or save it as an image. How to prepare a document for electronic signature? Fill in the form in a text file editing program. Save the document in PDF format. Sign the PDF file with a qualified signature or trusted signature. Send the electronically signed PDF file in accordance with the rules specified in the form. How to save a Word document as PDF? In Microsoft Word: Select File → Save As. Indicate the location to save the file. In the "Save as type" field, select PDF (*.pdf). Click Save. Alternatively: Select File → Export. Click Create PDF/XPS Document. Select the location to save the file. Click Publish. How to save a document as PDF in other programs? Most document editors (e.g. LibreOffice Writer, OpenOffice Writer, WPS Office, OnlyOffice) allow saving a document to PDF format. The most commonly used methods: Select File → Export as PDF or Export to PDF. Indicate the location to save the file. Click Export or Save. If the program does not have an export to PDF function: Select File → Print. As the printer, select Microsoft Print to PDF (Windows) or Save as PDF. Click Print. Indicate the location to save the file and save the PDF file. Important Before saving or printing the document to PDF, make sure that: all required form fields have been filled in, the appropriate checkboxes have been marked, the document layout is correct in the print preview. Verification of electronic signature Before sending the form, it is worth checking whether the signature has been correctly applied. After opening the signed PDF file, the PDF handling program should display information about the presence and status of the electronic signature. The absence of such information may mean that the document has lost its electronic layer and the signature will not be possible to verify. Note In the case of sending the form electronically, it is recommended to transmit the document directly in the form of a signed PDF file. Preserving the electronic layer of the document is a condition enabling confirmation of the validity of the qualified signature or trusted signature. Most common errors causing the form to be invalid marking more than 12 candidates, failure to fill in the organization's data, failure to check the declaration of operating in the territory of Warsaw, lack of signature of the person or persons authorized to represent the organization, signature made by a person not authorized to represent the organization, sending the form after the deadline, inability to identify the person making the handwritten signature. Note: In the case of joint representation, the form should be signed by all persons required in accordance with the manner of representation of the organization disclosed in the relevant register.
